With Grace provides Level 1 and Level 2 Support Coordination tailored to your needs. Whether you’re new to the NDIS or looking to get more from your plan, we’re here to make things clearer and easier.
We start by listening, learning about your goals, routines, and what’s important to you. Together we map out supports that fit your life, then coordinate providers and keep everything on track. You’ll have a dedicated coordinator who checks in regularly and is easy to reach when things change.
We provide face-to-face Support Coordination across Melbourne and virtual services throughout Victoria. If you’re in regional Victoria and prefer in-person catch-ups, we’re happy to organise periodic face-to-face meetings where possible.
Get in touch with us and we’ll confirm your Support Coordination funding and level. With your consent, we’ll collect plan details, book an initial meeting, and create a simple action plan to get your services underway.
Level 1 (Support Connection) helps you kick-off and use your plan. Level 2 (Coordination of Supports) provides ongoing coordination for more complex needs—working with multiple providers, resolving issues, and adjusting supports over time.
You get a single point of contact who understands your goals, keeps providers aligned, monitors outcomes, and helps you build capacity to manage supports more independently where that’s right for you.
Yes. We can step in to resolve service gaps, address conflicts, find replacement providers, and prepare evidence for plan reviews when circumstances change.
