NDIS Support Coordination (Levels 1 & 2)

Support Coordination for Empowered Living

Our Support Coordination helps you understand your NDIS plan, connect with the right services, and build the confidence to manage supports independently. We focus on personalised, high‑quality assistance so you can make the most of your funding and reach your goals at your pace.

Level 1 – Support Connection

Short‑term help to understand your plan and start using it. We explain your funding, introduce quality providers, and support you to make bookings so you can begin services with confidence.

Level 2 – Coordination of Supports

Ongoing, hands‑on coordination for more complex needs. We build a strong support network, monitor services, troubleshoot issues, and adapt supports as your goals or circumstances change.

What this includes

  • Explaining your plan and budget
  • Exploring provider options and quality checks
  • Setting up services, agreements and bookings
  • Problem‑solving barriers and resolving issues
  • Building skills to self‑manage where appropriate
  • Preparing for plan reviews with clear reports

How we work with you

We start by listening, learning about your goals, routines, and what’s important to you. Together we map out supports that fit your life, then coordinate providers and keep everything on track. You’ll have a dedicated coordinator who checks in regularly and is easy to reach when things change.

Where we offer services

We offer in-person Support Coordination in Melbourne and the Pilbara, and can connect with you virtually anywhere across Victoria or Western Australia.

Frequently Asked Questions

How do I get started?

Get in touch and we’ll confirm your Support Coordination funding and level. With your consent, we’ll collect plan details, book an initial meeting, and create a simple action plan to get your services underway.

What’s the difference between Level 1 and Level 2?

Level 1 (Support Connection) helps you kick‑off and use your plan. Level 2 (Coordination of Supports) provides ongoing coordination for more complex needs—working with multiple providers, resolving issues, and adjusting supports over time.

How does Level 2 Support Coordination benefit me?

You get a single point of contact who understands your goals, keeps providers aligned, monitors outcomes, and helps you build capacity to manage supports more independently where that’s right for you.

Can you help in challenging situations?

Yes. We can resolve service gaps, address conflicts, find replacement providers, and prepare evidence for plan reviews when circumstances change.
