With Grace provides Level 1 and Level 2 Support Coordination tailored to your needs. Whether you’re new to the NDIS or looking to get more from your plan, we’re here to make things clearer and easier.

  • Level 1 - Support Connection. Short-term help to understand your plan and start using it. We’ll explain your funding, introduce quality providers, and support you to make bookings so you can begin services with confidence.
  • Level 2 - Coordination of Supports. Ongoing, hands-on coordination for more complex needs. We’ll build a strong support network around you, monitor how services are working, troubleshoot issues, and adapt supports as your goals or circumstances change.
  • What this includes: explaining your plan and budget, exploring provider options, setting up and coordinating services, developing service agreements and bookings, problem-solving barriers, building skills to self-manage where appropriate, and preparing for plan reviews with clear reports.

How we work with you

We start by listening, learning about your goals, routines, and what’s important to you. Together we map out supports that fit your life, then coordinate providers and keep everything on track. You’ll have a dedicated coordinator who checks in regularly and is easy to reach when things change.

Where we offer services

We provide face-to-face Support Coordination across Melbourne and virtual services throughout Victoria. If you’re in regional Victoria and prefer in-person catch-ups, we’re happy to organise periodic face-to-face meetings where possible.

How do I get started?

What’s the difference between Level 1 and Level 2?

Level 1 (Support Connection) helps you kick-off and use your plan. Level 2 (Coordination of Supports) provides ongoing coordination for more complex needs—working with multiple providers, resolving issues, and adjusting supports over time.

How does Level 2 Support Coordination benefit me?

You get a single point of contact who understands your goals, keeps providers aligned, monitors outcomes, and helps you build capacity to manage supports more independently where that’s right for you.
