Social Work Services
At With Grace Support, we’re here to walk alongside you during tough times and big changes. Our social workers provide practical, hands-on support and evidence-informed guidance to help you manage daily challenges, strengthen connections, and move toward your goals with confidence.
We support young people, autistic people, and anyone experiencing physical disability or mental health challenges. Whether you need help with emotional regulation, daily routines, connecting with services, or navigating a crisis, our social workers offer steady, compassionate support for you and your family.
We offer face-to-face social work services throughout Melbourne and virtual services across Victoria. If your NDIS plan includes Coordination of Supports (Core and/or Capacity Building) or you’re self-funded, we’re here to help.
We provide practical support, counselling-informed guidance, advocacy, service linkage, transition planning, crisis and safety planning, skill-building for daily life, and family/carer support to keep things stable at home and in the community.
Anyone experiencing change, stress, or barriers to accessing supports—young people, autistic people, people with disability or mental health needs, and families seeking coordinated, practical help.
Contact us with your details. We’ll discuss your goals, confirm funding or payment options, complete an initial assessment, and create a simple plan with next steps.
We focus on building stability and confidence—supporting communication, routines, and access to services—so your household feels more connected and prepared for what’s next.
Face-to-face across Melbourne and online across Victoria, with periodic in-person catch-ups for regional participants when helpful.
